Special AT-rich binding protein 2 (SATB2) is an immunohistochemical marker for osteoblast differentiation. Our aim was to investigate SATB2 expression in oral osteosarcoma and other bone-producing oral tumors/reactive lesions to evaluate its usefulness as a diagnostic marker. Forty‐eight pretreated osteosarcoma biopsies, including 26 whole‐section cases and 22 tumours on tissue microarrays, and 36 non‐osteogenic bone sarcomas were evaluated. Forty‐five of 48 (94%) osteosarcomas showed nuclear immunoreactivity for SATB2 (all whole‐slide sections showed expression). Conclusions: SATB2 is a sensitive marker for osteosarcoma; however, it is not specific, with expression being observed in other high-grade primary bone sarcomas. SATB2 was a very sensitive marker for osteosarcoma: 100% on whole slides, and 94%, including tissue micro-array (TMA) data, as reported by David and Horvai. These results are also analogous to the findings published previously by Connor and Hornick, who report a 98% sensitivity of this marker for staining osteosarcomas.

SATB2 is a marker of osteoblastic differentiation in benign and malignant mesenchymal tumours. Although SATB2 is not specific for osteosarcoma, it has the potential to be a useful adjunct in some settings, particularly in the distinction between hyalinized collagen and osteoid. SATB2, strongly expressed in oral osteosarcoma, fibroosseous lesions, and central giant cell granuloma, is not a reliable diagnostic marker. However, SATB2 can distinguish between induced neoplastic bone and reactive bone tissue. Targeting SATB2 as an alternative therapy should be investigated. SATB2 is commonly expressed in osteosarcomas.
